    LIFE Inc. – Core Services
    1. Information & Referral
    Provides individuals with information about community resources, disability-related services, and supports that promote independent living.

    2. Independent Living Skills Training
    Offers training to help individuals gain or strengthen skills needed to live independently, such as budgeting, cooking, transportation use, and self-advocacy.

    3. Peer Support
    Facilitates opportunities for people with disabilities to connect, share experiences, and support one another through peer mentoring and group sessions.

    4. Advocacy
    Supports both individual and systemic advocacy efforts to protect the rights of people with disabilities and improve access to services, housing, and public accommodations.

    5. Transition Services
    Assists individuals transitioning from:

    -> Youth to adulthood
    -> Institutional settings to community living
    -> At risk of institutionalization to independent alternatives

  • Perinatal Health Partnership

    The Perinatal Health Partnership (PHP) Program provides home visiting services during pregnancy and up to the infant’s first birthday following delivery. Pregnant women with high-risk conditions or who have identified risk factors that put them at risk for poor pregnancy outcomes will be targeted for home visiting services and interventions but enrollment can occur after delivery. We are currently accepting all women who are pregnant at this time that are needing assistance while pregnant or post partum.

  • Safe Harbor Street Outreach

    Safe Harbor Street Outreach provides resources to homeless individuals or families through the Coordinated Entry system.
